INDEMNITY FORM
I wish to join the Course/ Training organized by ETANN Consultants Pte Ltd and abide by the Rules and Regulations by ETANN Consultants Pte Ltd. I am fully aware of the possible risks involved and accept the same, notwithstanding the fact that this course is intended only for those without medical problems and who are fit enough to indulge in physical activities. I confirm that I am enrolling on my own volition and I shall not hold the ETANN Consultants Pte Ltd, its servants and organizers responsible or in any way liable for my death, injury, disability or any loss or damage whatever arising from any cause in connection with the course or my participation therein. I hereby indemnify and agree to keep the ETANN Consultants Pte Ltd, its management, servants and organizers of the event fully indemnified against all claims, lost or damage whatever in respect of death, injury, disability or any loss or damage whatever arising from any cause in connection with the course or my participation therein.
Personal Data Protection Act Consent Form
- In compliance with the Personal data protection act, we seek your consent to collect and use your personal data (e.g Name, NRIC, FIN, contact number, mailing address, email addresses and photograph) for the purpose of and in accordance with the ETANN Consultants Pte Ltd personal data policy. A hard copy of the Personal Data Policy statement can be downloaded from our website (https://www.etannedusg.com/_files/ugd/a71bca_91b24d37797f48cbb9510eb5eb0c5b90.pdf). I confirm that I have read and understand the ETANN Consultants Pte Ltd Personal Data Policy Statement and do hereby give my acknowledgement and consent to ETANN Consultants Pte Ltd to use my personal data in accordance with the ETANN Consultants Pte Ltd Personal Data Policy.
TERMS & CONDITIONS:
* PDPA: Students to sign off on the PDPC Notice on the 1st day of class. The information provided contains personal data and is collected, use and disclosure for the purpose of processing the application and meeting SSG funding requirements.
* Trainee shall be bound by the Terms and Conditions of any applicable funding scheme as approved by SSG. If the trainee fails to meet any of the requirements set under the funding scheme (as stated below or otherwise) or has been granted funding for the same course before, as there by resulting that his/her application for funding claim is rejected by the respective funding agency due to any reasons, the trainee is liable to pay the balance of the course fee to Etann Consultants Pte Ltd.
* Trainee achieves less than the minimum attendance of 75% required by the course.
* Trainee fails to sit for the required examination/ assessment (where applicable).
* Trainee fails the required examination/ assessment (where applicable).
* Trainee absent without a valid Medical Certificate.
* Trainees who are using SkillsFuture Credit & are eligible for funding, if you fail to complete your course including assessment within 60 calendar days from the course end date or fail your assessment more than 3 times, you are required to pay the full course fee. (applicable to Singaporeans & SPRs)
* Student must achieve 75% course attendance and also to pass the Assessment to be eligible for SSG funding. The assessment is conducted by external government agency.
* Refund Policy:
* If trainee cancels registration 7 calendar days or more before course commencement date – 100% Refund. ? If trainee cancels registration 6 calendar days or less before course commencement – NO REFUND.
* If trainee does not turn up – NO REFUND.
* If trainee defers training/ assessment for more than 14 calendar days – NO REFUND.
* ETANN reserves the right to cancel or postpone the training session where enrolment is insufficient, or due to unforeseen circumstances.
* Company/ student must complete all procedures stated in the TPGateway portal to be eligible for SSG funding. In the event of failed course funding by SSG, company/ student has to pay ETANN full Course Fees.
* One Time Registration Fee: $18
* Administration fee: $30 will be charged if trainee withdraw less than 07 days before the class date. Re-Assessment Fees
* Basic Security Courses: $100 ($75 is for the assessment & $25 is for the pre-revision fee)
* For Senior & Supervisor Courses: $165 ($140 is for the assessment & $25 is for the pre-revision fee)
* For PAPP Course: $100 ($75 is for the assessment & $25 is for the pre-revision fee)
* For CSSPB Course: $165 ($140 is for the assessment & $25 is for the pre-revision fee)
* Assessment attire: Assessment attire: Smart and Formal attire. (Eg: White security collared shirt/ Formal shirt and dark coloured pants with covered shoes) NO JEANS AND SLIPPERS ? Classroom: Informal office attire
